There are some issues with people not being able to load images, specifically in Europe and The Netherlands. I've contacted Edgecast (the network used to display the images) and they're aware of what's happening and are working to get it fixed.

No, we don't recycle the names. Each image is has a unique file name that's checked against both available and deleted images.
They don't. They just convert it to JPG.
There was a partial outage with Imgur's delivery network (Edgecast). Everything is back up and running though.
